Job DescriptionQUALIFICATIONS:REQUIRED: Graduate of an accredited school of nursingCurrent RN licensure in MinnesotaCurrent RN licensure in Wisconsin within two months of hireCurrent BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptableStrongly demonstrated written and oral communication skillsPREFERRED: One year full time employment as an RNPHYSICAL REQUIREMENTS:Ability to sit or stand for prolonged periods at a time. Must possess speech, hearing and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently. Proficiency in other languages may be required. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.Performance is up to standard when:Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plansUses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their careEngages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care interventionIdentifies and secures patient services needed for patient careInforms and supports patients and families to assist them in making informed decisions and navigate the healthcare system.Education: Definition:Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding.
