We are seeking dedicated and compassionate Licensed Vocational Nurses (LVNs) to join our dynamic home health team in West Los Angeles. This opportunity is available for both Per Diem and Full-time positions. As an LVN, you will play a critical role in providing exceptional care to patients in the comfort of their homes.

Role Responsibilities

• Conduct home health visits and follow-up care for patients with various medical needs.
• Deliver skilled nursing care and prescribed medical treatments in accordance with the Nurse Practice Act of California.
• Collaborate with a supervising Nurse Practitioner or Physician to ensure quality patient care.
• Perform basic medical care tasks such as changing bandages, administering medications, and collecting specimens.
• Assist patients with daily activities including bathing, feeding, and dressing to ensure their comfort and safety.
• Monitor patients' conditions and report any changes to the supervising nurse.
• Support Registered Nurses in educating patients and their families about health management and care plans.

Qualifications

• Active LVN license in the state of California.
• Minimum of one year of clinical experience is required.
• Valid driver’s license with a clean driving record.
