A very well known social services organization is looking for a part or full time LPN Case Manager to join their growing team in Bronx, NY! 

Job Function:

\n

• Day Shift
\n
• 9am - 5pm
\n
• Provides direct and supportive healthcare to individuals that reside in IAHD's residential programs
\n
• The position focuses on coordinator of care/case management for individuals and staff training oversight for the provision of care
\n
• Documenting individual's status, health needs and follow-up
\n
• Attending staff meetings and participating in company functions as needed
\n

\n

Job Requirements:

\n

• New York State LPN License
\n
• Experience in direct clinical work with developmentally disabled populations preferred
\n
