SUMMARY

Join our dedicated team as a Home Health Care Aide, where you will provide compassionate and reliable care to clients in the comfort of their own homes. You will follow personalized care plans and make a meaningful difference in the lives of our patients every day.

STATUS: Full Time, Part Time

LOCATION: Clifton Springs, NY

DEPARTMENT: ElderONE

SCHEDULE: M-F Eve/Night, Sat & Sun Day, Eve, Night

RESPONSIBILITIES

• Patient Care & Service: Support and enhance patients’ health by executing tasks from care plans, collaborating with healthcare professionals, and assisting with personal care, light housekeeping, meal preparation, and errands.
• Communication: Effectively share patient health status, treatment details, and progress with the office and supervisory team.
• Documentation: Ensure accurate and complete records of client care on activity sheets.
• Compliance: Follow department protocols, HIPAA regulations, and local, state, and federal requirements diligently.

EDUCATION:

LICENSES / CERTIFICATIONS:

Home Health Aide certification from the New York State Education Department (NYSED) and a valid New York State Driver's License from the New York State Department of Motor Vehicles (NYSDMV).

PHYSICAL REQUIREMENTS:

Medium Work - Able to exert 20 to 50 pounds occasionally, and/or 10 to 25 pounds frequently, and/or up to 10 pounds constantly to aid in moving objects. Requires frequent walking, standing, or squatting.
