Job Description

To provide a Community Health Nurse. This position will offer clinical and educational nursing services to private and public agencies throughout the state and to coordinate and correlate such services with other services of the department and with various programs of private and other public agencies.

Works under the general supervision of a superior from whom a general program is received. This work is reviewed for conformance to departmental policies and professional practices.

Responsibilities:

• Offer clinical and educational nursing services to community agencies and health care professional partners throughout the state and to coordinate and correlate such services with other services of the department and with various programs of private and other public agencies for targeted population served in Refugee Health and other identified Division of Community Health and Equity.
• Coordinate the assessment of individual and family health in needs of promotion of preventive and health care services and to implement proper programs or treatment.
• Support with the monitoring and review of nursing services of private and public agencies and to evaluate the services provided in order to ensure program effectiveness and compliance with applicable professional, regulatory, and contractual standards and requirements.
• Assist various agencies in developing quality assurance programs to ensure the delivery of such quality program services for population served in Refugee Health and other identified Community Health and Equity programs.
• Conduct studies, surveys, and on-site visits of health care facilities for the purpose of determining the assessment of services and to prepare reports thereon.
• Communicate with health care partners and community partners ongoing referrals and care coordination needs.
• Apply the nursing process including assessment, planning, implementation, and evaluation in the care of clients and work with the Consultant Medical Directors review ongoing requests.
• Communicate health, medical and nursing information to other members of the interdisciplinary team and health care contracted partners.
• Serve in a liaison capacity between health care partner agencies, community partners and the Department and other concerned state agencies in planning, coordinating, implementing, and evaluating community health services.
• Interpret nursing and health care needs to other members of the interdisciplinary team.
• Monitor health status of patients/clients and support with coordination of care for notified medically complex cases.
• Maintain records and prepare written reports as required.
• Perform related work as required.

Education and Experience:

• Education gained through graduation from an accredited school of nursing and/or graduation from a college of recognized standing involving a program of study in nursing; and
• Experience gained through considerable employment providing professional nursing services involving clinical and/or educational health work.
• One to three years' experience working with community organizations, and state agencies.
